I checked out the waiver guide after not having reviewed it in a long time.  Here's the URL:

The document stated that the last review is 13 Jun 08.  Looks like there is some new guidance.  In particular I saw that civilian applicants for UPT/UNT can go to civilian providers for refractive surgery (PRK / LASIK).  I saw that on pg 469-470, did I read that right?  That's quite a change from when I went through it.
